How to access school led tutoring (SLT) forms
You can access the school led tutoring (SLT) form using your linked DfE Sign-in account.
Deadline to complete the form is Friday 14 October 2022.
How to link your DfE sign-in account
If you need to access the form and your DfE sign-in account isnt linked, then you will need to complete these steps:
- Login to your DfE Sign-in account
- Click on ‘organisation’
- Select ‘request access to an organisation’
- Enter ‘URN or UKPRN’ of the organisation you want your account linked to
- Select the organisation from search results
- Select ‘confirm’
You can link your account to several organisations if required.
A request will be sent to the approver of that organisation. Once approved, your account will be linked to the organisation.
You will then be able to access the SLT form through DfE sign-in.

How to resolve access issues not related to accounts being linked
If you are experiencing access issues not related to accounts being linked, you will need to follow these steps:
- Please sign out of your DSi account completely
- Clear the cache and cookies
- Close and re-open the browser
- Attempt to access the form via this link for either schools or Local Authorities
You can also try a different web browser, WI-FI and different machine to try and resolve the issue.

- Create an email titled “SLT access issue”.
- Take screenshots, capturing the address bar and include a link to the form you are trying to access.
- Email screenshots of the journey you take to get into the system including any error messages to enquiries.esfa@education.gov.uk
