Apprentice change of employer paperwork

Hi all,

What paperwork do you all complete for a change of employer and what level of details do you include, ie original start date, start date with new employer, planned OTJ hours etc?  Would you be willing to share your paperwork with me?

All paperwork as it is a new employer, add original start date only if a restart/

I know we wouldn't add original start date in the ILR, but could this be included in the paperwork to make it easier for working out remaining funding etc, OTJ hours for new employer?

On the commitment statement with the new employer you would record in the APL section the training previously undertaken with the first employer.
